Your 5-1-7 Youth Cheer Roster is due by August 14th, 2026. Please submit this form after you have filled out the team registration form.

If you coach multiple teams, please submit one roster form for each team. This roster helps us keep track of participants for scholarship and insurance purposes.

If your school would like to be considered for hosting a 5-1-7 Youth Cheer competition, please complete this form.

Understanding Score Sheets

Score sheets and penalty sheets need to be filled out, saved as a PDF and emailed to the host school no later than NOON the day BEFORE you compete, or you are subject to a penalty for submitting late paperwork.

Score and penalty sheets need to be saved as a PDF to ensure that all information stays in the necessary spaces on the sheets. Other formats may adjust your paperwork to be incorrect.

How to Open Score and Penalty Sheets:

Open the file found at the button below
Click Make a Copy. You will then be asked to rename the file, please use your school and team name. It will save to your own Google Drive.
You will be able to make edits to the sheets before sending them to the host schools prior to a cheer meet.
